OUH Patient Values Survey

The following survey includes questions about your experience being cared for at OUH and how you would like things to improve.

The survey should take no longer than five minutes to complete and the information you provide will help us to define our culture moving forwards.

The information you provide in this survey will be treated confidentially. We ask some questions about you, such as your age, gender, ethnicity and other demographic information, to help us understand whether people from different backgrounds have different experiences and to make sure our services are fair and inclusive.

Your responses will be analysed and reported in a way that does not identify you. Demographic information will only be used to look at patterns and themes across groups of respondents and will not be used to identify individuals.
