1.Please enter Name of Camper or Leader-in-Training (LIT) and his/her/their contact information (Please use separate surveys for multiple campers/LITs).
2.What school does your Camper attend & what is current grade?
Virtual Young Teen Asthma & Wellness Camp (ages 11-15, 1-week, June 22-26, 2026 ALL VIRTUAL).
LIT (ages 15-18) Training Monday June 3 (Virtual) then serve as a Group Leader During Camp.

Orientation sessions (Virtual) will be available for Camper/LITs/Parents/Guardians who register.
