Youth Squad 360 Survey

Hello,

 

If you were a Summer Night Lights Youth Squad member from 2008 to 2015, we want to hear from you!

 

To help make a lasting, positive impact in the lives of Summer Night Lights Youth Squad Members, The GRYD Foundation is developing an expanded level of service called Youth Squad 360, representing 360 degree wrap around service. 

 

The objective of The GRYD Foundation Youth Squad 360 Initiative is to help SNL Youth Squad members achieve long-term stability and success at home, at school and in the workplace.  

 

We hope this survey help us understand what services would be most helpful to you as you pursue your school and employment goals. Your honest answers will help us to better understand what are the actual needs as we develop Youth Squad 360 programming.

 
The survey: This survey should take no more than 10 minutes to complete. The survey asks about your prior role with the Summer Night Lights program, your current situation, and what services you think would help you achieve your school, life and work goals.

There are minimal risks to participating in this survey. You will be asked questions about how you are doing, what you feel you have achieved since being employed as a Summer Night Lights Youth Squad member, and what program services you think would be helpful to you. Some of these questions may make you feel uncomfortable, because of personal experiences. If you do not wish to answer a question, you may withdraw and not complete the survey.

Anonymity:  Any reports resulting from this survey will not identify you by name as a participant. All information gathered in this survey will remain confidential and be given out only with your permission or as required by law.
