The Home Centered Care Institute (HCCI), with support from the Elea Institute, is thrilled to introduce the Chronic Care Management Innovation Award, an 8-month initiative designed to empower Illinois-based palliative care organizations. This program aims to help participants diversify their services, enhance patient care, and drive sustainable revenue through the implementation of Chronic Care Management (CCM).

Eligibility Criteria:
- Established community-based palliative care programs operating in one or more of the following Illinois counties: Boone, Cook, DeKalb, DuPage, Kane, Kendall, Lake, LaSalle, Lee, McHenry, Ogle, Will, Winnebago
- Certified Electronic Health Record (EHR) System currently in place or within 30 days of Innovation Award acceptance
- Program provides 24/7 patient access to care, defined as offering on-call services including after-hours coverage, so the patient has access to clinical advice and guidance
Expectations:
- Commitment at the C-Suite level to implement CCM upon the conclusion of the program, including necessary actions & deliverables to support a successful implementation
- Adherence to Centers for Medicare & Medicaid Services (CMS) requirements and guidelines

- Interested programs are strongly encouraged to research CMS requirements for delivering CCM services to determine the program’s feasibility
- Execute Participation Agreement and HCCI Business Associate Agreement (BAA)
- Complete a Post-Engagement Survey
- Complete Pre- and Post- Assessments to measure progress
