Welcome to my survey
Introduction
This is an Annual Survey for people affected by sight and dual sensory loss across Leicester, Leicestershire and Rutland.
Thank you for your interest in our annual survey. If you are visually impaired in any way, or support someone who is, this survey is for you.
Your views will help us understand what matters to you most, and the findings will help us understand if people are getting the support they need, as well as the improvements that are needed.
By sharing your experiences, you’ll strengthen our work to advocate and influence change locally as well as raise awareness of sight loss and the work we do, as the charity providing support for local people with sight and dual sensory loss.
How long will the survey take?
· About 15 minutes
· We ask you to complete in full in one go, by pressing submit at the end of the survey. Exiting before this will lose your answers.
· Some questions you must complete, others you can skip and leave blank if you wish.

How will we use the data in this survey?
The data is anonymous, and will be analysed and produced in a report that will be shared on our website and crucially used to influence local providers of care and support.
