Parent Survey

The Jefferson County Drug Free Coalition is conducting a short survey for parents or guardians of youth ages 18 years and under to assess their knowledge and behaviors related to youth alcohol, tobacco, and other drug use. This survey will take approximately 5 minutes to complete.

**Youth is defined as anyone ages 18 years and younger**
