I, the undersigned volunteer [“Volunteer”] of the Youth Pride Chorus of South Florida (YPCSF), a program of the Gay Men’s Chorus of South Florida (GMCSF), agree and understand that the purpose of GMCSF and YPCSF is to advocate for the LGBTQ+ community and its allies. I hereby release GMCSF and its directors, staff, and agents from any and all claims, responsibility, liability, or causes of action, for any injury, loss, or damage that I may incur in connection with my volunteer activities at YPCSF and GMCSF.

I further understand that, as a volunteer of YPCSF and GMCSF, any and all information pertaining to participants, patrons, and donors is strictly confidential. I agree to hold in confidence any information about participants, patrons, and donors, which comes to my knowledge during my association with YPCSF and GMCSF. I understand and acknowledge that either party may terminate this volunteer relationship at any time and that submitting this application does not imply a guarantee to volunteer. Upon acceptance as a volunteer, I agree to serve under the leadership, guidance, and procedures of GMCSF and the YPCSF program for the duration of my volunteer service. I agree to abide by all safety policies established by GMCSF and the YPCSF program. I will uphold the mission, vision and values of YPCSF and GMCSF. I understand that GMCSF  and the YPCSF program may at its sole discretion adjust policies o from time to time as needed to meet the evolving needs of the program. I also understand that my failure to comply with policies and procedures and/or the direct requests of staff and designees may result in the termination of my volunteer relationship with YPCSF and GMCSF.
