Family Voice Surrey Educational Needs and/or Disability annual parent carer survey (2026).

Welcome to the Surrey Special Educational Needs and/or Disability (SEND) Annual Satisfaction Survey 2026. This survey is for parent carers of children and young people aged 0–25 years with Special Educational Needs and/or Disabilities (SEND). It gives you the opportunity to share your experiences of education, health and social care services over the last 12 months.

If you have more than one child or young person with SEND, you may complete the survey separately for each child.

The survey should take approximately 15–20 minutes to complete.

If you need support to complete the survey or would like a paper copy, please contact Family Voice Surrey at contact@familyvoicesurrey.org.

Your responses are anonymous. Individual responses will only be accessed by Family Voice Surrey and will not be shared, unless a response raises a concern about someone’s safety.

The results of this survey will be used to help improve SEND services in Surrey. An anonymised summary of the findings will be shared with Surrey County Council and relevant NHS partners. A full report will be published later in the year.

Because this survey is anonymous, it is not suitable for raising individual complaints or safeguarding concerns. If you have safeguarding concerns, please contact Surrey County Council directly by phoning 0300 470 9100.
CONSENT STATMENT

By providing consent, you are agreeing that personal information about you will be collected anonymously, stored securely and will not identify anyone personally. The information from the survey will be used to understand the current strengths and potential areas of improvement in Surrey. The anonymised, thematic data will be used and shared with Surrey County Council and relevant NHS partners. Once consent has been provided you have the right to withdraw your views and your views will be removed, however where data has been anonymised and amalgamated into thematic responses in certain circumstances it may not be able to be excluded. Thank you for taking the time to complete this survey.
