Community COVID-19 Vaccine Survey

Erie Family Health Centers wants to hear from our communities about what kind of information people want as they make their decisions about the COVID-19 vaccine. Thank you very much for taking just a minute or two to answer a few questions. At no point will you need to give us your name, so you should feel free to be as honest as possible.
