BWC: Transitional Housing to Rapid Re-Housing Survey

You are invited to participate in a research study conducted by Robin Petering, PhD, MSW. Your participation is voluntary. You should read the information below, and ask questions about anything you do not understand, before deciding whether or not to participate. You may also discuss participation with your family or friends. 


The current evaluation project proposes to assess the impact of the BWC: Transitional Housing to Rapid Re-Housing Survey. 


If you volunteer to participate in this study, you will be asked to complete a short survey (~30 minutes) that will ask you questions specifically about the Hope Home program and other questions related to your experiences with housing and homelessness, income and education; health and well-being; early childhood experiences; social connections; and service utilization. At the completion of the survey, you will be compensated with $20 cash or a gift card of equivalent value.

We will also ask you to participate in a follow-up interview over the phone with a research assistant. This interview will ask you more specific questions about the Hope Home Program. It will be audio-recorded and then transcribed. At the completion of the interview, you will be compensated with $20 cash or a gift card of equivalent value.


Some of the questions might be upsetting to you since they ask about things like past childhood events that might have upset you. You can choose to skip any question or stop the survey/interview at any time. If you feel upset, you can talk to the person who gave you this survey to get resources to help.


Any identifiable information obtained in connection with this study will remain confidential. The identifiable information that you provide will not be directly connected to any data throughout study. Your name or other identifiable information will be stored on a secure computer that only the PI, Robin Petering, has access to. All data will be stored on a password protected server at Lens Co. 

However, if you disclose any information regarding child abuse or intent to harm yourself or others, the researcher must notify agency staff, potentially DCFS or the police, and the faculty advisor for this study.

After we are finished with the study, we will destroy any files that are linked to your name. The contact information will be deleted. We will keep the de-identified data in our database for an indefinite period of time and it may be used in future research studies. If you do not want your data to be used in future research, you should not participate.

When the results of the research are published or discussed in conferences, no identifiable information will be used. Reports of the findings will be shared with program partners but data will be shared in an anonymous/aggregate form such that no individual respondents can be identified.

We will do everything we can to keep others from learning about your participation in this study.


Your participation is voluntary. Your decision not to participate will not involve any penalty or loss of benefits to which you are otherwise entitled. You may withdraw your consent at any time and discontinue participation without penalty. You are not waiving any legal claims, rights or remedies because of your participation in this research study.
