Thank you for participating in Northreach Society's 2SLGBTQ+ Youth Needs Assessment Survey. If you have any questions or concerns, please contact amanda.g@northreach.ca. This project is funded by Women and Gender Equality Canada in an effort to explore the unique and specific needs of 2SLGBTQ+ youth living in Grande Prairie. The survey is open to all youth between the ages of 13 to 29 years who identify as 2SLGBTQ+.

Instructions:

- Some questions within the survey are marked as mandatory for survey consistency purposes. If a question that is marked mandatory does not apply to you or you have no answer, please fill it in saying "not applicable" or "not relevant" (or something similar).

- Demographic questions are asked for tracking purposes as well as for intersectional impacts, but please note there is a "prefer not to answer" and an "other" option if you are not comfortable answering or you feel the list does not reflect your identity. We recognize lists do not tell the full story.

- There are 3 verification/security questions being asked. These are not meant to confuse genuine participants but are bot deterrents as this was an issue previously. These questions are mandatory but not scary!

- Once completed, if you want to be entered to win a $200 Gift Card, please complete the second form. This is optional and not mandatory. 10 chances to win, one entry per person, fraudulent forms not accepted, chance to win depends on amount of surveys completed but will be capped at 200, winners will be notified within 2 weeks of survey closing.
