The South East Grey Community Health Centre is now offering the Client Experience Survey online. This confidential portal will provide us with your instant feedback. A paper copy, and assistance (if needed), is available at each of our CHC sites. After completion of the survey, you can enter to win one of six $50.00 gift certificates at Foodland. Ballots are available onsite or simply email info@segchc.ca with the subject line: Survey Complete
