Primary Care Alternative Payment Model Measure Selection Tool

This survey allows Primary Care Medical Providers (PCMPs) to select measures for the Primary Care Alternative Payment Model.

To earn the full enhancement available in Fiscal Year (FY) 2020-21, PCMPs will need to earn a score of at least 190 points from no more than 10 measures.

95 - 142                                                     2% - < 3% 
143 - 190+                                                3% - 4%+ 

 

 

PCMPs should select the measures on which they would like to work on. To do this, check the box next to the selected measure (labeled “Select Measure”). The available measure set includes:
(1) structural measures
(2) electronic clinical quality (e-CQM) performance measures
(3) claims performance measures

In the following pages you will be able to select structural, e-CQM and claims based performance measures (each with their points listed adjacent). Please recall that you can select no more than ten measures in total (across the three measure sets). The ten selected measures need to total or exceed 190 points. So please be sure to keep a tab on number of measures and points selected on each page.
