WI FACETS Ambassador Program: Volunteer Application

Thanks for your interest in becoming a Parent Ambassador for WI FACETS. The Ambassador program provides leadership building opportunities for volunteers to reach out to parents and families in their communities or statewide to educate and empower.

The questionnaire below consists is a set of 12 questions that will allow us to know more about you and your interest to support our mission of enhancing the lives of children and young adults with disabilities and their families.

We would love for you to take part of the FACETS’ family. Your talents and time will make a difference in the lives of Wisconsin families with children with disabilities that don’t know yet about our services and/or benefit from connecting with others. By completing this application, you will be contacted about the next steps and to invite you to participate in a 1-hour virtual orientation to learn more about FACETS’ programs and how the Ambassador opportunity can fit your desire to reach out to other families of children with special needs.
