Introduction
Thank you for taking part in the survey and your interest in using the new toolkit that is being developed.
The following is a consent form for taking part in the evaluation process and questionnaire to help us develop a wider bladder symptom toolkit for people with parkinsonism. The hope is for this to be made widely available later this year.
This form contains information on the study, followed by an online consent form. Once this is completed, you will be given a link to the main questionnaire.
In the near future when available, the first draft of the toolkit will be provided to you to use in your clinical practice. We would then be very grateful if you could complete another questionnaire about your experiences to help us further refine this.
