This year as part of the 2023 TCCN Quality Program, we are asking that all practices commit to quality improvement activities and coding for the 2023 TCCN Measures of Focus which are listed below: 
  • Well child 0-30 months​
  • 3-21 Well Child Visits​
  • Asthma
  • Behavioral Health and Depression Screenings
  • BCBS and Amerigroup Contracted Measures

Transition to adult care

The process of transitioning a pediatric patient to adult care must occur at a certain point in a patient’s life.  With your assistance, we are gathering information to evaluate when this occurs for our practices and if there is an established plan. This will help us evaluate our patients and membership needs.  For a typical patient please answer the following questions.
