1. Sports Health Questionnaire Introduction
The Summa Center for Sports Medicine delivers athletic training and sports medicine services to the athletes of our region. Our goal is to enhance the quality of health care for the physically active of all ages and advance the sports medicine services provided.
We appreciate the working relationship that we have with your community and want to continue to improve on our services.
In order for us to continue to do so, we need honest feedback to tell us how we are doing.
