Senior Survey 2016

Each year seniors provide information regarding their awards, accomplishments, and future plans.  This information is used in the preparation of the Senior Award ceremony, for state reporting, and to plan future academic and extracurricular opportunities for students.  Completion of this survey is required.  Completion of the survey takes approximately 30 minutes.  Most students find it helpful to have a parent/guardian assist them during the process.  You will be asked to provide ALL scholarships you are eligible to access.  This includes scholarships offered to you from schools you do not plan to attend.  Please collect this information prior to completing the survey.  You may visit this page again to add additional scholarships.  You will need to fill out all required fields each time you visit the survey.  If you have any questions about this survey you may contact YHS Guidance office.
