Practice Application


Thank you for your interest in the Montana Chapter of the American Academy of Pediatrics Adolescent Vaccination & Wellness ECHO!

The Montana Chapter of the American Academy of Pediatrics (MTAAP) Adolescent Vaccination & Wellness ECHO serves as a forum for clinics serving patients 11-21yo, to develop and implement best practices in addressing vaccination barriers by addressing workflows, cultural and social considerations, and vaccination hesitancy training. This program connects primary care providers with a team of multidisciplinary experts, including nursing and front office staff, to address how to overcome barriers to regularly scheduled adolescent vaccines.
 
Participants will meet for six lunchtime sessions April-July for a brief lecture followed by a de-identified patient or process case presentation and discussion.

The application deadline is April 20, 2021; however, applications will be accepted on a rolling basis with first come, first served.

Sessions meet on the Thursdays at 12:10-1:10PM MST on April 29, May 6, May 20, June 3, June 17 and July 1.

ECHO sessions occur virtually using Zoom video conference technology.

Eligibility criteria:
  • Provide primary care to adolescents ages 11-21.
  • Identify a core project team that will provide leadership and oversight within the practice for the project. The team may include, at a minimum, a primary care physician who will serve as physician lead; a nurse; and/or an additional team member such as front office staff, practice manager, or care coordinator. Practices may choose to include additional members on the core project team, and all clinic staff can participate in educational
    offerings.
  • Agree to the participation requirements:
    • Participate in a minimum of 5 of the 6 live ECHO sessions
    • Complete individual pre-post project evaluation surveys
    • Complete monthly PDSA cycles
    • Submit Case Form
This application can be completed by the physician or nurse project champion or a designee. Each practice will be eligible to receive $500 upon completion of the project if they meet CME/MOC requirements to use toward clinic signage and implementation of items that overcome barriers to vaccination hesitancy.
