The purpose of this survey is to gather some preliminary information about resources that are helpful in supporting the use of individualized funding. We look forward to hearing about the resources you use or know of for individualized funds. This project is funded by the Ontario Ministry of Community and Social Services.
The ultimate goal is to make these resources easily available to anyone who wants to use them through The IF Library for individualized funding. The IF Library is intended to be useful to adults with DD, their families/support networks, and agencies.

The survey will take less than 10 minutes of your time. Your responses will remain anonymous.

None of the services or funding you or anyone you support use will be affected.
