Investigators: You are invited to participate in a project led by student investigator Haya Fernandez as part of a Master’s thesis under the supervision of Dr. Tejal Patel and in collaboration with Dr. Lora Giangregorio, Dr. Martin Cooke and Dr. Kelly Grindrod from the University of Waterloo and Cynthia L. Milburn, the executive director of Epilepsy South Central Ontario.

Neither the investigators in this project nor the institutions are receiving payment for doing this research.

Purpose of the study: The primary objective of the study is to determine the proportion of adults aged 50 and older with epilepsy who take vitamin D supplements.

Procedures: Patients who are diagnosed with epilepsy and are aged 50 years and older are invited to participate in this study.

If you agree to participate in this study, you will be asked to complete a questionnaire that will take approximately 20 minutes to complete.

In this questionnaire, you will be asked about your supplement use, diet, and bone health.

You will be also asked to provide some demographic information such as age, sex, ethnicity, frequency of seizures, and current and past use of antiepileptic medications.

Possible benefits of the study for me and/or society: Participating in this study might enhance participants’ awareness of their bone health.

Possible risks and discomforts: There are no anticipated risks.
