Hello 2022 Section Officers:

Please fill out the following form to report on your section's activities and finances for 2022. You may return to this form at any time by following the link in the email sent to you. We recognize that not all of the standard questions will pertain to you. If you did not have any activities this year, please respond as such.

This form has been adapted to follow the new Section Funding Guidelines. We ask that you do the best you can to provide complete information, but understand that this is a transitional year. Moving forward, Officers will be expected to submit the complete information requested. Please email Annie Imperatrice, imperatrice@siam.org with questions regarding the form.

The deadline to submit your report is January 31, 2023. Failure to return this report in a timely fashion can affect your section's eligibility for funds.
