The LMC has recently received feedback from a number of practices regarding the delivery of neighbourhood community mental health services across City & Hackney. Whilst some practices report excellent working relationships with their local teams, others have highlighted challenges relating to communication, liaison arrangements, referral processes and continuity of contacts.

The feedback received so far suggests there may be significant variation in how neighbourhood mental health services are operating across different areas. In order to better understand the current picture and identify both areas of good practice and opportunities for improvement, the LMC is seeking feedback directly from practices.

The information collected will help us to:
• Understand the consistency of service delivery across neighbourhoods.
• Identify examples of effective practice that could be shared more widely.
• Better understand any challenges relating to communication, liaison arrangements and referral management.
• Support constructive discussions with local mental health services regarding future service development and improvement.

We appreciate that practices are extremely busy, therefore we have kept the survey deliberately brief and would be grateful if one representative from each practice could complete it.
