Youth Community Roundtable: Parent and Youth Registration & Consent Form

General Information 

Join the California Department of Food and Agriculture Office of Farm to Fork (CDFA-F2F) for a virtual Youth Community Roundtable. 

 

The roundtable is open to youth who are in middle or high school and who have an interest in school food. 

 

There will be two community roundtable sessions, with 30 youth participants in each session. By completing this form, you are signing up for the session held on:

- Thursday, January 28, 2021 from 4-5pm PT 

If you wish to participate, please complete this form. After completing and submitting the form, you will receive the Zoom link and joining information. Guidelines for participation will be provided at the beginning of the Zoom call. 


What do I need to participate? 

- Complete this registration form, including signed consent by your parent or guardian
- Access to Zoom
- Willingness to share your perspective
- Openness to hear from others


What will I be asked? 

We want you to share your ideas and perspectives on topics like: 

- Things you think about when deciding what to eat 
- What you do and don’t like about school food 
- Items you would like to see on your school’s breakfast or lunch menu 
- Your ideal school meal 
- What you would like your school cafeteria to look and feel like 
- Your ideal school lunchtime experience 

CDFA-F2F will share a summary of youth ideas and perspectives with the California Farm to School Advisory Group, which is working to create a unified vision and roadmap for school food and food education in California. 


Additional Information 

Participation is voluntary. There will be a few guiding questions, but mostly we hope to create a space that enables honest discussion and understanding. 

The conversation is meant to be informal. The conversation will not be recorded. After the conversation, CDFA-F2F will summarize the ideas and perspectives youth discuss (youth names and any identifying information will be kept confidential) before sharing these perspectives with the California Farm to School Advisory Group. 


Consent to Participate 

Consent to participate is required below by both the youth participant and the parent or guardian. 

This form is to be completed together by the youth participant and the parent or guardian. 

If either the youth or parent/guardian wishes to revoke this consent for any reason, please promptly notify CDFA-F2F at cafarmtofork@cdfa.ca.gov.
