Community Health Needs Assessment

We want to hear from you!

In 2018, Community HealthCare System surveyed area citizens to determine health needs. Now we ask for your input again to help us create a 2021 Community Health Needs Assessment for Pottawatomie County. Your responses are confidential. All input is valued and helps us understand how to best serve our patients and communities. Thank you for your help!

Note: This survey is for those who either live or work in Pottawatomie County. If you neither live nor work in the county, please exit the survey now. The deadline to participate is November 22. The survey has 29 questions and will take only a few minutes to complete.
