Informed Consent Form (Part 1)



Introduction

The title of this research study is Understanding the Relationship Between Diabetic-Related Health Knowledge, Psychosocial Health Efficacy, and Diabetic Control.

I am Christiana Etukudo, a doctoral student under the supervision of Dr. Bryan in the College of Doctoral Studies at Grand Canyon University.

The purpose of this study is to seek to know the relationship between diabetic-related health knowledge, psychosocial self-efficacy, and the likelihood of diabetic control. Several studies have been conducted looking into the subject of the link between diabetic-related health knowledge, self-care management, and diabetes control. However, more still needs to be known about how diabetes affects the diabetic population in northern California.

This document defines the terms and conditions for consenting to participate in this research study.

How do I know if I can be in this study?

o   You can participate in this study if you:
·       Are aged between 18-65 years.
·       Diagnosed with Type 2 Diabetes 6 months ago and above.
·       Live in northern California.
·       Are under the supervision of a Licensed Physician.
·       Are taking 2-3 diabetic medications.
·       Male, Female, and others are inclusive.

o   You cannot participate in this study if you:
·       Are not diagnosed with Type 2 diabetes.
·       Are younger than 18 years or older than 65 years.
·       Are not living in northern California.

Research Activities: What am I being asked to do?
If you agree to be in this study, you will be asked to:

o   What?
·       Participate in the study by completing a survey questionnaire that will take you approximately 20-30 minutes to complete.
·       Agree to an informed consent form that confirms your approval to participate in the study.
·       The survey will be open from November 6, 2023, through December 6, 2023.
·       You will not provide your name, place of work, or any other identifiers in the survey responses. Therefore, your responses cannot be linked back to you.
·       Your participation in this research is voluntary, and there is no financial cost or payment for participating.
·       You can leave the survey at any time without any penalty.
·       Please note that this study is not expected to pose any risk to you if you choose to participate.

o   When?
·       The survey will be open from November 6, 2023, through December 6, 2023.
·       The survey is limited to the Type 2 diabetes population in the nine counties in northern California.

o   Where?
This survey will be conducted online only via SurveyMonkey.
