Thank you so much for your interest in participating in this online survey, which aims to identify the experiences and challenges of women with secondary lymphedema and the interventions that are provided to you. You are being asked to participate in this study to discuss your experiences and challenges with secondary lymphedema in order to direct the creation of a prototype sleeve that can assist in the treatment of lymphedema.  

Currently there is no gold standard intervention for patients with secondary lymphedema. This study could lead to the creation of a sleeve that could provide that type of intervention for the treatment of secondary lymphedema.  

Should you decide to take part in this survey, there is a risk that someone could trace the information in a central database back to you.  The researchers believe the chance that someone will identify you is very small, but the risk may change in the future as people come up with new ways of tracing information.  

The results of the study may be released to the funding agency Louisiana State University Board of Supervisors, the LSU Health-NO Institutional Review Board, University Medical Center New Orleans (UMCNO) and the investigators. Records will be kept private to the extent allowed by law.  

Organizations that may inspect and/or copy the survey for quality assurance and data analysis include: the sponsor, The Louisiana State University Board of Supervisors, the LSUHSC-NO Institutional Review Board, University Medical Center New Orleans (UMCNO) and the investigators. 

By answering the questions and submitting the survey, you are agreeing to participate. You can skip any question you do not feel like answering or that makes your uncomfortable.
