2.Enter the name of 5 exhibitor booths you visited.(Required.)
3.Enter the 5 exhibitor "Secret Words" in order of exhibitor names entered above.(Required.)
4.Please enter your email to qualify for the draw.(Required.)
RULES/INSTRUCTIONS:

-Additional survey entries can be completed for each 5 exhibitors visited
-Conference Sponsors, HIMSS Board Members, HIMSS Conference Committee Members cannot participate.
-Surveys must be submitted by 3:20pm in order to qualify.
-The draw will take place during the closing on October 30.
