Oakville Hornets 2015/2016 Mid Term Rep Parent Survey

This is a mid term survey to allow the for the organization to acquire early feedback as to the development of our players and coaches. It serves as a critical input guide to our goal of making the Oakville Hornets Rep program as successful as possible.We continuously strive for improvement with the program and this tool will help us in enabling that. We will use the results to help us provide feedback to our coaches, to enable us to make adjustments to our development programs and to give parents an opportunity to share their year to date experiences with us.
All parent surveys will be tabulated confidentially and all individual responses will be viewed by the VP of Rep.
We will forward only summarized information to the coaches if requested. The identity of all respondents will remain confidential.
Please include your name and phone number so that concerns might be discussed if you choose and the Vice President of Rep(Patrick OKelly) will be able to contact you if necessary
