Welcome to our survey! You are responding as a participant in the At the Intersection of Service Provider Wellbeing and Equity in Healthcare and Social Services Systems Transformation project funded through Vancouver Foundation in partnership with Bridge For Health Cooperative and the Public Health Association of BC. We seek to discover how you as a direct service provider define and experience personal wellness in the workplace. We see personal joy, a sense of fulfillment, and the opportunity to connect and communicate within the organization especially as it relates to sensitive conversations centered on aspects of diversity (e.g., race, gender identity, sexual orientation, etc.) as fundamental to the experience of wellness. To that end, we wish to offer our perspectives of various terms used throughout the survey to help provide context.

We want to ensure we hear from a range of voices, experiences and lived realities when it comes to service provider well-being. Our goal is to make this survey accessible and we acknowledge and are mindful that barriers do remain (i.e., language barriers as this survey is being provided in English, access to technology, preference for oral communication, screen fatigue, etc.). We would love to still hear from you if you require accommodation and to make this process easier for you. If you would prefer a phone call or alternative methods in completing this survey, please reach out to us at survey@bridgeforhealth.org.
