The purpose of the document is to determine your school’s interest in SRTS activities and degree of readiness to continue participating in the Safe Routes to School(SRTS)program in the next school year.
Below you will find questions that will help identify your school’s areas of interest and degree of readiness to participate in the next phase of the Sonoma County SRTS program during the 2016-2017 school year.
Please note that only those who complete this questionnaire will be considered for admission into the program for the 2016-17 school year.
