Children's information

Please provide your child's information for our records
1.First Child's Information(Required.)
2.Allergy/Medical Information: Are there any allergies and/or medical conditions the MFRC should be aware of? If yes, please give detail including symptoms, severity of condition, if your child carries an EpiPen or inhaler and any other details that would be of assistance.(Required.)
3.Behavioural Information: Are there any behavioural concerns the MFRC Staff should be aware of? If yes, please give detail of the concerns and how the MFRC staff can assist.(Required.)
4.Photo Release: Photographs and video may occasionally be taken at Youth Programs. Photographs and videos may potentially be used in any MFC publication including print, website, or other electronic form of social media.(Required.)
5.Off Property Release: Do you grant permission to Shilo MFRC Youth Programs, to take your child on local outings including but not limited to: walks in wooded surrounding base, Bunker Hill, Canex & parks in residential areas.(Required.)
6.Release Information (for children in Kindergarten-grade 6): Please indicate how your child will be getting home at the end of the program.(Required.)
If you selected "Picked up do NOT let walk home", you will need to pick your child up from inside the MFRC. If you would like to meet outside, you will need to select walking or biking.
Should you need to change release information, please speak to one of our Youth Programs Staff Members.

Children in Kindergarten-grade 6 will be required to stay for the duration of the program unless we are otherwise advised by their parent.
Youth in Grades 7-12 will be allowed to come and go from the Centre at their own discretion.
