This form is to collect Affiliate team nominations for the 2021 QLD Junior State Cup. 
 
Only the Affiliate organiser can complete this form and you must have approval from your Affiliate prior to submitting the team nominations. 
 
Please include all teams that your Affiliate wishes to nominate for this years event.

Once your nomination has been received you will receive confirmation from the event manager and be provided access to the Queensland State Cup Event within the Mysideline platform where you can create and manage you Affiliate teams within the system.
