Distinguished Teacher Nomination Form
The purpose of the Colorado Teacher Award is to recognize the teaching profession by honoring distinguished teachers for their excellence in teaching. Last year’s recipient was Math teacher, Dave Frantom.
Staff, students, and parents are needed to nominate teachers for this prestigious award. Please consider nominating an eligible teacher. For those of you who are new to Cherry Creek Schools, the recipient receives recognition from December through May at our school, in the community, and at the District Teacher Awards Banquet.
We encourage colleagues, students, and students’ parents of the following teachers to submit a nomination form on behalf of a teacher who has touched their life in a special way. You may nominate more than one teacher (please use separate forms).
CRITERIA: Currently a full-time classroom teacher
Has taught in the CCSD for five full years
Has not previously received this award at any other Cherry Creek school
