Why Take This Survey?
Every other year the Autism Society of California gathers information on what is working and not working for families and individuals with intellectual and developmental disabilities (ID/DD) - this includes autism, intellectual disabilities, epilepsy, and cerebral palsy.
This information will be shared with nonprofit organizations, stakeholders and legislators to ensure that the community's voice is heard, new programs and services can be developed, and policymakers have a chance to learn what the needs are in our community.
All personal information will be kept confidential. Information obtained will be used in aggregate to identify issues and concerns for the community. If a quote is used, we will do so anonymously or change the name of the individual providing the quote.
If you have more than one person in your family with an intellectual or developmental disability, you may take the survey multiple times.
If you choose to do so, you can enter your name at the end of the survey for your chance to win one of our $100 AMAZON Gift Card. You must complete the survey by Oct 31st to be entered into the drawing. Winners will be notified on Nov 5 , 2018.
