Dear Committee Chairs and Staff,

In an effort to develop a means to evaluate your committee's initiatives or events AFTER they take place, the following evaluation form has been created. Please utilize this evaluation form to share the results of your committee's initiative or event identified in the CBI 2017-2020 Strategic Plan. 

Also note that if you have more than one initiative or event to evaluate, kindly complete a separate survey for each initiative or event.  Please include all relevant information, sharing specifics about preparation as well as any concerns you experienced.  Including specific information in the comment boxes for each question will be particularly helpful. The information gathered in all evaluations will play an instrumental role in determining the success of 2017-2020 CBI Strategic Plan as well as the future direction of Congregation Beth Israel.
