This scholarship provides financial support to individuals participating in the Transplant Games of America.
Applicants must complete the form below and submit their story through one of the listed options.

  • I understand my Transplant Games of America registration status will be verified
  • I understand that, if selected, I will be required to participate in the Transplant Games of America
  • I agree to share my experience through a testimonial, video, or social media to be featured on The Decision Project® platforms
  • I agree to participate in a TDP event or feature by August 8, 2026
