EMPHN has been funded by the Department of Health, Disability and Ageing to connect residents living in aged care homes with a regular, visiting GP.
The purpose of this survey is to identify:
- Residential aged care homes that require increased GP services
- GPs that have capacity to take on new residents
Upon completion, an EMPHN staff member will be in touch to discuss next steps within 1-2 business days.
