PARENT AND PARTICIPANT RELEASE AND WAIVER MAY 18, 2020

After reading this Release & Waiver carefully, please sign below to indicate your agreement:

Mansfield Soccer Association (d/b/a Revolution Soccer Club) will resume soccer activities June 1, 2020 as allowed by the State of Texas under the guidelines set forth on May 18, 2020. Revolution Soccer Club will follow the rules, regulations, and policies of the Office of the Governor of the State of Texas. As these policies are subject to change, so are those of Revolution Soccer Club. Please keep in mind that attendance at all soccer events, or participation in all soccer events, and the use of facilities and equipment, are at your own risk.

To participate in soccer events (herein “events”), parents should monitor the health of their children and not send them to soccer events if they exhibit any symptom of COVID-19. They should seek COVID-19 testing promptly and report results to the program (mansrevsc@gmail.com) given the implications for other children, families, and staff. Individuals aged 65 or older are at a higher risk of COVID-19. Parents should protect any vulnerable persons who are members of the same household or come into frequent, close contact with individuals who participate in youth sports.

Symptoms of possible COVID-19 include:
 Cough
 Shortness of breath or difficulty breathing
 Chills
 Repeated shaking with chills
 Muscle pain
 Headache
 Sore throat
 Loss of taste or smell
 Diarrhea
 Feeling feverish or a measured temperature greater than or equal to 100.0 degrees Fahrenheit. If measuring under the armpit, a fever is constituted at 99.0 degrees or higher.
 Known close contact with a person who is lab confirmed to have COVID-19
 Please note that public health guidance cannot anticipate every unique situation. Individuals should stay informed and take actions based on common sense and wise judgment

The virus that causes COVID-19 can be spread to others by infected persons who have few or no symptoms. Even if an infected person is only mildly ill, the people they spread it to may become seriously ill or even die, especially if that person is 65 or older with pre-existing health conditions that place them at higher risk. Because of the hidden nature of this threat, everyone should rigorously follow the practices specified in these protocols. The virus that causes COVID-19 is still circulating in our communities. We should continue to observe practices that protect everyone, including those who are most vulnerable.

It is recommended to follow health guidelines, including:

 Washing hands with soap often and for 20 seconds or use hand sanitizer with at least 60% alcohol
 Cover coughs and sneezes with a tissue, then throw the tissue away.
 Avoid touching your eyes, nose, and mouth with unwashed hands.
 Practice social distancing, avoid crowds, and limit physical contact.
 Use simple cloth face coverings in public.

I UNDERSTAND THAT THERE ARE CERTAIN RISKS INHERENT IN THE GAME OF SOCCER. I ALSO UNDERSTAND THAT MY ATTENDANCE AT THE EVENTS, OR PARTICIPATION IN THE EVENTS, AND MY USE OF EVENTS FACILITIES AND EQUIPMENT MAY BE DANGEROUS AND COULD INVOLVE THE RISK OF SERIOUS INJURY, EVEN DEATH. IN LIGHT OF THE COVID-19 PANDEMIC, I ACKNOWLEDGE THAT (A) ANY LEVEL OF INVOLVEMENT OR PARTICIPATION IN THESE KINDS OF EVENTS MAY EXPOSE ME TO VARIOUS VIRUSES OR INFECTIONS THAT COULD BE DANGEROUS OR EVEN DEADLY; (B) I AM RESPONSIBLE TO MAKE MY OWN DECISIONS REGARDING SOCIAL DISTANCING AND THE USE OF PERSONAL PROTECTIVE EQUIPMENT; AND (C) IN THE EVENT OF INCLEMENT WEATHER, I MAY HAVE TO CHOOSE BETWEEN TAKING SHELTER IN A LOCATION THAT DOES NOT ALLOW SOCIAL DISTANCING, INCLUDING BEING TRANSPORTED IN A GOLF CART OR OTHER VEHICLE OR BEING EXPOSED TO LIGHTNING OR OTHER WEATHER-RELATED DANGERS.
