Covid -19 Screening Checklist

This checklist provides basic information only. It is not intended to take the place of medical advice, diagnosis or treatment.
1.Do you have any of the following symptoms: fever/feverish, new or existing cough & difficulty breathing?
2.Have you travelled internationally within the last 14 days (outside of Canada)?
3.Have you had close contact with a confirmed or probable Covid-19 case?
4.Have you had close contact with a person with acute respiratory illness who has been outside of Canada in the last 14 days?
If you have answered NO to all of the above questions, then you have passed the Covid-19 screening & may enter the office. 

If you have answered YES to any of the above questions, or have refused to answer any of the above questions, then you have not passed the Covid-19 screening & may not enter the office. Please contact your local Public Health Unit or Telehealth Ontario for further instruction.
