Thank you for coming to the 2022 SANYS Conference Award Nomination Form.

You can nominate someone you know for one of the awards by answering the questions below.  The awards are as follows:  Self-Advocate of the Year, Self-Advocacy Group of the Year, Advisor of the Year, Direct Support Professional of the Year, and Agency of the Year. 

 Also, please note the following nominations will not be eligible to be a nominee for an award:

-  Nominations from people who nominate themselves,

-  Nominations from a family member who nominate another family member,

-  Nominations of SANYS Board members or staff.

You can nominate in one category, or multiple categories, but please make sure you include a description of why you think someone deserves an award for everyone you nominate.

* 1. The Bernard Carabello Self-Advocate of Year Award is for a person who has worked hard to speak up for themselves and others and has done something that has improved the community for other people with developmental disabilities.
