Mental Health Partners/Providers: Emergency & Crisis Services Survey

Greetings, 211 is working to develop a list of mental health providers offering services to those affected by the July 4th mass casualty event in Highland Park.

The following survey aims to assess and establish a list of providers available to assist and what types of services they offer.

211 will provide the information collected from you in this survey to the community as they reach out to us for resources via the 211 hotline and on the 211Now website Highland Park Parade Shooting Resources. If, after completing this survey, you need to make changes or remove your services, please email us at 211lakecounty@icfs.org.
