Thank you for your interest in the Focus on HARM (Harmonizing Accountability in Reporting and Monitoring) Initiative from the National Quality Forum (NQF).

NQF will be convening experts with experience in patient safety event review, classification, reporting, coding, or related health IT experience to support the first stage of the initiative: Updating the Serious Reportable Event List. The Technical Expert Panels (TEP) will be divided into two groups of experts: (A) one TEP will review candidate Serious Reportable Events and develop consensus for inclusion, and the other (B) will establish implementation guidance for the included events.

In this survey you will be asked to provide the following:
  • Survey page 2 - personal information and CV or resume
  • Survey pages 3 and 4 - brief explanations about your expertise and interest related to this project
The approximate term for experts will be from June 2024, through February 2025. Monthly meetings will be convened tentatively starting in July and extending thru September; after September, time commitment will be minimal.
