Dear Study Participant,
My name is Bethany Kurina and I am a Doctoral Candidate in the Counseling Psychology Program at Carlow University. The following survey will be used to gather data for my dissertation. The purpose of my dissertation study is to explore psychologists’ relationships with their older adult clients.
Participation Criteria
If you are a licensed psychologist who works with at least one adult client 65 years of age or older, please consider participating. Please take your time in reading this document before deciding if you would like to participate. If you wish to retain a copy of this informed consent form for your information and record, please print this page.
If you do not meet the criteria for this study please consider passing the survey along to psychologists you know who may meet the study criteria.
Participation Procedure and Confidentiality
Your participation in this study will be limited to the completion of a demographic questionnaire and surveys about you and your relationship with your older adult client. The survey should take you 20-30 minutes to complete. While completing the survey, you will not be asked to provide any identifying information. Therefore, your responses will be completely anonymous. In addition, participation in this study does not provide the researcher access to your online profiles. All responses are confidential, and results will be kept on the primary investigator’s password protected computer. Your participation is voluntary and you may withdraw from this project at any time.
Your voluntary response to this request constitutes your informed consent to your participation in this study. You are not required to participate. If you decide not to participate, your decision will not affect your current or future relations with Carlow University.
Risks and Benefits
There are no foreseeable risks associated with completing the survey nor are there any direct benefits to you.
Costs and Compensation
There are no costs to participate in this study. To thank you for your participation, each participant will have the option to be entered into a drawing to receive one (1) of four (4) $50 Visa gift cards. If you would like to be entered into this drawing, once you click “submit” at the end of survey, you will be taken to a new webpage not associated with your survey so as to maintain your anonymity, and you will be asked to provide information for the drawing. The drawing will be held after the completion of data collection. The winner will be notified via email.

This study has been approved by the Carlow University Institutional Review Board. This Committee administers both the General Assurance of Compliance with the United States Department of Health and Human Services Policy for the protection of Human Subjects and the University policy covering the protection of human subjects.
