ADA Transition Plan Survey

The City of High Point is completing an ADA Transition Plan.  This plan will look at barriers to accessibility and strategies to address a variety of issues that impact the accessibility of City services, facilities, sidewalks and streets in the public right-of-way. This survey is intended to inform the ADA Transition Plan update by generating priorities for improving City-owned infrastructure throughout the municipality.

INSTRUCTIONS:

Please fill out the short 11 question survey on accessibility. It should take approximately 5-7 minutes to complete. “Walking” refers to rolling, pushing, walking, jogging, using mobility devices or any movement in the public right of way not designated for vehicular traffic specifically.
