This questionnaire is for the parents who have been using our Before and/or After School Care Program. It has been designed to allow the staff at OSCA to gain an understanding of your and your children’s opinions and attitudes regarding the service we provide. We ask you, therefore, to please have your say so that we can make this service better suit your and your children’s needs and interests.
