* 12. All players not including reserves should be available to play at least 50% of matches.

Players must pay their membership fees before or after this form is submitted. Failure to pay membership fees within 14 days once invoiced will deem this application invalid and the applicant not permitted to play.

Fees are final and per competition as follows:
All Night/Twilight Tennis $80 for 3 player & $70 for 4 player teams per player (covers cost of lights, balls and Tennis West Registration fees)
All Day Doubles Competition $60 per player (covers cost of balls and Tennis West Registration fees)
All Day Singles/Doubles Competition $70 per player (additional can of balls, external court hire due to lack of courts and Tennis West Registration fees).
Submitting the form acknowledges the fee and agreement that payment will be made for the invoiced amount.

Home teams are expected to provide hospitality for their opposition team after the matches are completed.

If reserves are required initially or added later to compile a full team then payment may not be required until the end of the season once all matches have been played. Players playing the minimum rounds to qualify may then be charged on a matches played basis.
