The St. Lucie County Community Health Improvement Plan (CHIP) is intended to be used as a roadmap for collaborative action over the next five years rooted in the collective vision established by the Healthy St. Lucie Steering Committee during the Community Health Assessment (CHA) process.

After nearly a year of collaboration through the CHA and CHIP cycle to turn data into dialogue, insight into action, and partnerships into progress, CHIP Implementation is driven by the Priority Area Workgroups.

Workgroups will meet monthly (online or in person) to collaborate on activities. These groups will also provide progress updates at the Quarterly CHIP meetings. Please indicate your interest in participating in a workgroup by completing the form below.
Vision: A Healthy St. Lucie is one where all residents, from youth to seniors, feel heard, supported, and connected to resources and spaces that advance their well-being. In a Healthy St. Lucie, there are opportunities to enrich the lives of all residents through systems that are easy to navigate, led by a united front of stakeholders who focus on up-stream approaches.
