Title of Study: Exercise Attitudes and Exercise Habbits

Purpose of the Study
This study focuses on exercise and how people’s experiences and attitudes effect their exercise habits, especially as they age.
 
Study Procedures
If you agree to take part in the study, it will require you to answer several questions. Most of these will be multiple-choice regarding exercise routine, attitudes toward exercise, body image, openness, and experiences with primary care givers. The first questions will ask you to recall a memory of physical activity and to answer some questions about this memory. It is estimated that this survey should take between 30 minutes and an hour. Approximately 70 volunteers will participate in this study. Participation is entirely voluntary, and you may withdraw from the study at any time with no penalty to you.
 
Potential Benefits
From volunteering in this study, you may gain some insight into your own level of physical fitness. Beyond this, there are no particular benefits to you in completing this survey.
                          
Foreseeable Risks
This project is unlikely to have more than minimal risk, or any risk greater than that encountered in your routine daily activities.
 
Costs to You
There will be no costs to you for participation in this research study.
 
Compensation
If you are part of this study, you will receive no compensation or payment.
 
Confidentiality
You will not be identified in any reports on this study. Results of the study will be reported in aggregate form only. Records will be kept confidential to the extent provided by federal, state, and local law. However, the Institutional Review Board, the sponsor of the study, or university and government officials responsible for monitoring this study may inspect the records.
