1. About this survey

Please read the informed consent information below before completing the survey.

Informed Consent for Survey

We would like to invite you to participate in a research study. The purpose of this research study is to look more closely at women's experience with breech birth and pregnancy. We are interested in knowing how women feel about breech pregnancies and the choices available to them.

We are inviting you to be in this study because you have experienced a breech pregnancy or birth.

If you agree to participate, we would like you to complete one or several of the surveys, as many as are applicable to your situation. Each will take approximately 15-30 minutes to complete. If you do not wish to participate, you do not need to complete or submit the following survey. You are free not to answer any of the questions on the survey.

We will keep the information you provide confidential; if we write an article about this study we will do so in such a way that you cannot be identified. We have not placed any identifying codes or marks on the survey. You do not need to provide any identifying information or contact information in order to participate in the study. You may skip any questions you do not wish to answer. You will not benefit personally from being in this study. However we hope that others may benefit in the future from what we learn as a result of this study.
